Focus Group Sign-in Sheet
The purpose of these focus groups is to collect valuable community feedback on the implementation, delivery, and outcomes of the Upper Digestive Cancer Project. The participants of this group will come together for an in-person sitting, and a facilitator from the Canadian Hub of Applied and Social Research will guide the discussion. The session will be audio-recorded for the purpose of generating a transcript for analysis.
Some data from this focus group will be shared with the Canadian Partnership Against Cancer (the Partnership), which has helped fund this initiative, and with Three Hive Consulting, the evaluation group contracted by the Partnership. This data may be used in the Partnership’s internal and external communications to help demonstrate the impact of this and other Models of Care funded projects across Canada.
It is expected that the session will last 90-120 minutes. Those who have attended information sessions, including healthcare professionals, patients, and their families, are encouraged to attend; however, all members of the community are welcome.
The sessions dates are to be determined, but are expected to run in January/February of 2026.
Confidentiality
For confidentiality purposes, specific patient names should be omitted.
The audio/visual recording of your [
interview/focus group
] will be transcribed for analysis. Rev.com is a third-party platform used to support transcription.
Your audio will not be used to train Rev.com’s artificial intelligence.
Audio files are only kept on the system for the duration of transcription (3-5 minutes) and quality review check (up to approximately 3.5 hours). It is immediately and permanently deleted from the platform once the transcript has been reviewed.
It is recommended not to include any personal or sensitive information within the recordings transcribed with the third-party AI platform. However, if you do decide to disclose any of this information, it poses less risk to your privacy than posting on a public social media page.
